‘We’, ‘us’ and ‘our’ refer to TRS Plan Management. ‘You’ and ‘your’ refer to anyone whose personal information we collect and/or hold.

What information do we collect?

How do we hold personal information?

We take reasonable steps to maintain the security of your personal information and to protect it from unauthorised disclosures.

Electronic information is kept on secure servers and hard copy information is kept in locked filing cabinets when not directly being used.

How do we use your personal information?

The most important reason why we collect personal information is to be able to provide relevant, quality services and supports to you.

We may share your personal information to ensure NDIS funds are being used appropriately, such as if we have reason to believe a provider is invoicing for services they are not entitled to. We have processes in place to make sure only essential information is shared.

We do not share your information with any other business or service provider without your consent, unless we are compelled by law to do so.

Accessing or correcting your personal information

We are transparent and accountable for the personal information we collect and hold.

If you want to access or correct the personal information we hold about you, talk to your Plan Manager, or contact the General Manager on (03) 6326 1112 or email manager@trsplanmanagement.com.au.

Exceptions

We may share information about you without your consent when:

• The safety of others is at risk
